1. Introduction
The growing technology in the country, specifically in the Philippines, some establishments use technology, in order to keep their data, so whenever they want their particular information, it is easy for them to retrieve it. Usually big companies use a database management in order to keep their data. But not all enterprises use a system that will help their day to day transactions. Some of them still use a manual system, like some clinics and hospital. In gathering information about their patients, they still use a manual system, like keeping their patient’s record in a filing cabinet. And in order to retrieve the record of a patient, it is time cumbersome; there are chances that some records are loss. The researchers choose the Siasco Multi – Specialty Clinic as the basis of their proposed records monitoring system database.
They said that, they’re having a hard time retrieving old files and sometimes misplaces them; they also said that there are some instances that they keep the files in wrong divisions, and these files are not easy to look at because of many tests results attached to it. And lastly they added that they are running out of space in keeping the files of their patient in their clinic.
So the researchers come up a design, on how to keep records of a patient, with the use of a database management.
1.2 Statement of the problem
During the interview with Dra. Siasoco, she told the researchers that they are having a hard time in sorting all the files of their patients due to the growing number of their patients, also she added that they’re running out of space to place all the files of their patients.
